The Advance Project (Dementia) – Clinician/Manager

This course aims to provide an overview of advance care planning and palliative care for people living with dementia. This interactive, practical and self-directed course has been developed for aged and primary care professionals to explain how to use the Advance Project (Dementia) suite of resources with people living with dementia and their families. The Advance Project (Dementia) Clinician/Manager Course is designed for nurses, allied health staff and managers working in residential, home/community and primary care settings who are interested in learning and developing their skills to initiate advance care planning and palliative care needs assessment with people living with dementia.

We hope this course will help you feel more confident to initiate conversations about advance care planning and assess palliative care needs of people you are caring for who are living with dementia, as well as support their families/carers.

Equipment
To complete this course, you will need access to the following equipment:​
– A valid email address that is accessible from the computer you will be using to access the course.
– Stable internet with sufficient bandwidth to cope with streaming video-based resources.
– Computer software – PDF Reader, Video Player.
– Google Chrome is the preferred web browser for our Learning Management System.

Communication
The primary method for communication is via email. Once you have submitted the enrolment form you shall receive a confirmation of enrolment message and then an invitation to create an account with our Learning Management System, aXcelerate. Please ensure that you check your email Junk folder for these initial emails.

Duration and Access
This course will take you approximately 4 hours to complete.
You will retain your access to the course, all tools, resources, and support documents from the date of your enrolment until 29 April 2024.

Award
Certificate of Completion for 4 CPD Hours.

Following your completion of the course, you will be invited to complete some evaluation questions.
